What causes it
An erection depends on blood flow, nerve signals and hormones working together, so a problem with any one of them can cause difficulty. Physical causes include high blood pressure, high cholesterol, diabetes and the side effects of some prescribed medicines. Psychological causes include stress, anxiety and depression. Alcohol, smoking and tiredness all contribute.
Erectile dysfunction is sometimes the first sign of an underlying cardiovascular problem, so the questionnaire asks about your general health as well as your symptoms. If your answers suggest something that needs investigating, the prescriber will tell you to see your GP rather than issue a prescription.
Treatment options
The usual first treatment is a PDE5 inhibitor, a tablet taken before sex that increases blood flow to the penis. Sildenafil is the most widely used of these in the UK. It is taken about an hour beforehand and lasts around four hours. It does not produce an erection on its own; arousal is still needed.
It is not suitable for everyone. It cannot be taken with nitrate medicines, and it needs care if you have heart disease, low blood pressure, or have recently had a stroke or heart attack. Common side effects include headache, flushing and a blocked nose. For some people, lifestyle changes, treating an underlying condition, or addressing a psychological cause is more appropriate than medication, and sometimes instead of it.
