Topical Dutasteride Hair Loss Treatment

Topical dutasteride is available with or without minoxidil and in multiple strengths. Each 100ml bottle is freshly compounded in the UK using dutasteride (0.025–0.2%) in a low-alcohol TrichoSol base. Prescription included following a short online assessment.

How it works

Testosterone is converted into DHT by the enzyme 5-alpha reductase, which exists in two forms. Finasteride inhibits type II. Dutasteride inhibits both type I and type II.

Use

Apply 1ml once daily to a clean, dry scalp. Leave on for at least 4 hours.

What to expect

Hair grows in cycles, so any change is assessed over months rather than weeks. Reduced shedding is often the first sign, at around 2 to 3 months. Visible change, if it happens, is judged at 3 to 6 months with your prescriber.

Dutasteride is licensed for men. It is prescribed to women off-label, at the prescriber's discretion, and only where there is no possibility of pregnancy.

How to apply

1. Wash and dry your scalp before application (or apply to dry scalp between washes)

2. Apply 1ml to affected areas of the scalp once daily. A 1ml pipette is supplied with your bottle

3. Massage gently into the scalp with clean fingertips

4. Wash your hands after application

5. Allow the solution to dry before styling or sleeping (10 to 20 minutes)

6. Leave it on for at least 4 hours before washing your hair

Consistency matters more than technique. Missing days slows progress, particularly in the early weeks.

Side effects

Topical dutasteride delivers the drug directly to the scalp with lower systemic absorption than oral dutasteride, though systemic effects can still occur, particularly at higher strengths (0.1 to 0.2%). Dutasteride has a longer half-life than finasteride, so any effects take longer to resolve after stopping.

Exact frequencies are not established for topical dutasteride, and the rates quoted for the oral capsule do not carry across, so the list below is ordered by likelihood rather than precise figures.

More likely (local, at the application site)

• Mild scalp irritation, redness or itching after application

• Localised dryness or flaking (usually resolves once the scalp adapts)

Less likely

• Reduced libido

• Erectile dysfunction

• Changes to ejaculation volume

Unlikely (mostly associated with the oral capsule)

• Breast tenderness or enlargement

• Mood changes (incidence unclear, still reported)

• Persistent sexual effects after stopping treatment

• Reduced semen quality or male infertility, worth discussing with your prescriber if you are actively trying to conceive

• Testicular pain or discomfort

• Changes in liver function

When to seek urgent medical advice

• Signs of allergic reaction: facial or lip swelling, difficulty breathing, rash

• Significant mood change or thoughts of self-harm

• Unusual breast lumps or nipple discharge

Not suitable during pregnancy or breastfeeding. If you notice any side effect, contact your prescriber before stopping so the decision can be reviewed properly.

Warnings and precautions

• PSA lowering: Dutasteride can artificially lower Prostate Specific Antigen (PSA) blood test results by around 50%. If you have a PSA test in future for prostate cancer screening, tell your GP you are taking topical dutasteride so the reading can be doubled for interpretation.

• TrichoSol base: The propylene glycol free, low-alcohol vehicle reduces the scalp stinging and dryness commonly reported with standard dutasteride solutions.

• Post-finasteride syndrome: If you have previously stopped oral or topical finasteride due to persistent sexual, mood or physical effects, disclose this on the questionnaire before starting dutasteride.

• Concurrent DHT-blocking treatment: Do not start topical dutasteride alongside oral dutasteride, oral finasteride, or topical finasteride without prescriber guidance. Combining increases systemic exposure and side-effect risk.

• Fertility planning: If you and your partner are actively trying to conceive, discuss with your prescriber before starting.

• Longer half-life: Any side effects from dutasteride take longer to resolve after stopping than they would with finasteride, because dutasteride has a much longer half-life (weeks versus hours).

Combining topical dutasteride with other hair loss treatments
