How dutasteride works for hair loss

Dutasteride is a dual 5-alpha reductase inhibitor. It blocks both the Type I and Type II enzymes that convert testosterone into dihydrotestosterone (DHT), the hormone responsible for miniaturising hair follicles in men with male pattern hair loss. At the standard 0.5mg daily dose, dutasteride reduces serum DHT by 90 to 98%, compared with roughly 70% for finasteride.

Type I 5-alpha reductase is expressed in scalp skin and sebaceous glands. Finasteride leaves this pathway largely active, meaning a significant source of scalp DHT remains during treatment. Dutasteride closes both doors, which is why it is often reached for when finasteride has plateaued or the loss pattern is aggressive.

What to expect

• 2 to 3 months: reduced shedding is often the first sign

• 4 to 6 months: visible thickening and some regrowth on the crown and mid-scalp

• 12 months: full effect judged. Results maintained by continued daily use

• On stopping: any hair gained is lost over 6 to 12 months as DHT returns to normal. Dutasteride has a long half-life (roughly 5 weeks) so DHT levels take longer to recover than after finasteride

Dosing

One 0.5mg capsule daily, at any time of day, with or without food. Consistency matters more than timing. Do not open, chew or crush the capsule. If you miss a dose, take it as soon as you remember on the same day; do not double up the next day.

Side effects

Most men take dutasteride 0.5mg without issue. Sexual side effects are the most reported effects, though the reported incidence in trials is not dramatically different from placebo. Dutasteride has a much longer half-life than finasteride (weeks versus hours), so any side effect takes longer to resolve after stopping.

Uncommon (roughly 1 in 100)

• Reduced libido

• Erectile dysfunction

• Changes to ejaculation volume

Rare (less than 1 in 1,000)

• Breast tenderness or enlargement

• Mood changes (incidence unclear, still reported)

• Persistent sexual effects after stopping treatment

Very rare (less than 1 in 10,000)

• Reduced semen quality or male infertility (worth discussing with your prescriber if you are actively trying to conceive)

• Testicular pain or discomfort

Who should not take dutasteride

• Anyone under 18

• Anyone with severe liver disease should be reviewed carefully by a prescriber

Pregnancy and fertility

Dutasteride can affect a male foetus. The capsules are soft-gelatin coated, so simply taking dutasteride does not expose a pregnant partner. The risk arises only if a pregnant woman handles a leaking or damaged capsule. Handle the capsules with dry hands and do not split them.

Dutasteride can also be absorbed through skin, so blood donation from men on dutasteride is not accepted for at least 6 months after stopping. If you and your partner are actively trying to conceive, discuss with your prescriber, as dutasteride can reduce semen parameters in some men.

Warnings and precautions

• PSA lowering: Dutasteride can artificially lower Prostate Specific Antigen (PSA) blood test results by around 50%. If you have a PSA test in future for prostate cancer screening, tell your GP you are taking dutasteride so the reading can be doubled for accurate interpretation.

• Post-finasteride syndrome: If you have previously stopped finasteride due to persistent sexual, mood or physical effects, disclose this on the questionnaire so your prescriber can review carefully before starting dutasteride.

• Concurrent DHT-blocking treatment: Do not start oral dutasteride alongside oral finasteride, topical finasteride or topical dutasteride without prescriber guidance. Combining increases systemic exposure and side-effect risk.

• Longer half-life than finasteride: Effects and any side effects take longer to resolve on stopping, because dutasteride has a much longer half-life (weeks versus hours).

• Blood donation: Not eligible to donate blood while taking dutasteride or for 6 months after stopping.

Dutasteride vs finasteride

Finasteride 1mg is the licensed first-line UK treatment for male pattern hair loss. If it produces good results with no side effects, most men stay on it long term. Dutasteride is often chosen when:

• Finasteride has plateaued or produced only modest results

• The pattern of loss is aggressive and needs stronger DHT suppression

• The man specifically wants the strongest available medical option

Clinical evidence for dutasteride in hair loss

Dutasteride is the more thoroughly evidenced of the two 5-alpha reductase inhibitors when it comes to raw DHT suppression, though the licensed hair loss indication in the UK is for finasteride 1mg. The evidence base for dutasteride in male pattern hair loss:

• Olsen et al 2006 (Phase III, 416 men over 24 weeks): dutasteride 0.5mg daily produced hair count improvements significantly greater than placebo and comparable to (in some measures greater than) finasteride 1mg. Total hair count gain at 24 weeks was around 92 hairs per 2.54cm² area at the vertex on 0.5mg dutasteride vs 41 hairs on finasteride 1mg vs 3 hairs on placebo.

• Gubelin Harcha et al 2014 (Phase III South Korea/Taiwan, 917 men over 24 weeks): confirmed dutasteride 0.5mg superiority over finasteride 1mg for hair count and Global Photographic Assessment. This is the trial that supported dutasteride licensing for hair loss in South Korea and Japan (though not in the UK).

• DHT reduction: dutasteride 0.5mg daily reduces serum DHT by 90 to 98%. Finasteride 1mg reduces serum DHT by 70%. Scalp DHT reduction is similar in magnitude. The stronger DHT suppression is why dutasteride produces marginally better hair count outcomes in head-to-head trials.

• Half-life: dutasteride has a much longer half-life (roughly 5 weeks) than finasteride (roughly 6 to 8 hours). Effects and any side effects therefore take much longer to reverse on stopping. This is a clinical consideration when starting.

Practical takeaway: dutasteride is the escalation option when finasteride 1mg has plateaued or produced only modest results. Both are effective; dutasteride is stronger. Never take both together (our clinical rule: one DHT blocker at a time).

Regulatory status in the UK: BPH-licensed, off-label for hair loss

• Avodart 0.5mg is MHRA licensed in the UK for benign prostatic hyperplasia (BPH) since 2003. This is the licensed indication.

• For hair loss, dutasteride is prescribed off-label in the UK. The MHRA has never licensed dutasteride for androgenetic alopecia, unlike South Korea, Japan and a handful of other markets where a specific hair loss licence exists.

Storage and shelf life

• Store at room temperature (below 30°C), away from direct sunlight and moisture.

Excipients: what is in each Avodart 0.5mg soft capsule

Active ingredient: dutasteride 0.5mg. Avodart is a soft gelatin capsule, so the excipients are different from typical tablet fillers. Typical composition:

• Capsule fill: mono- and di-glycerides of caprylic/capric acid (a fatty vehicle that dissolves the dutasteride), plus butylated hydroxytoluene (BHT) as an antioxidant to keep the drug stable.

• Capsule shell: gelatin, glycerol, titanium dioxide, iron oxide yellow (E172) giving the capsule its characteristic yellow colour.

• Printing ink: shellac, propylene glycol, iron oxide red (E172) for the "GX CE2" branding on each capsule.

The capsule is NOT vegetarian or vegan because the shell contains gelatin. The full ingredient list is on the manufacturer PIL enclosed with your medicine.
