How dutasteride works for hair loss

Dutasteride is a dual 5-alpha reductase inhibitor. It blocks both the Type I and Type II enzymes that convert testosterone into dihydrotestosterone (DHT), the hormone responsible for miniaturising hair follicles in men with male pattern hair loss. At the standard 0.5mg daily dose, dutasteride reduces serum DHT by 90 to 98%, compared with roughly 70% for finasteride.

Type I 5-alpha reductase is expressed in scalp skin and sebaceous glands. Finasteride leaves this pathway largely active, meaning a significant source of scalp DHT remains during treatment. Dutasteride closes both doors, which is why it is often reached for when finasteride has plateaued or the loss pattern is aggressive.

What to expect

• 2 to 3 months: reduced shedding is often the first sign
• 4 to 6 months: visible thickening and some regrowth on the crown and mid-scalp
• 12 months: full effect judged. Results maintained by continued daily use
• On stopping: any hair gained is lost over 6 to 12 months as DHT returns to normal. Dutasteride has a long half-life (roughly 5 weeks) so DHT levels take longer to recover than after finasteride

Dosing

One 0.5mg capsule daily, at any time of day, with or without food. Consistency matters more than timing. Do not open, chew or crush the capsule. If you miss a dose, take it as soon as you remember on the same day; do not double up the next day.

Side effects

Most men take dutasteride 0.5mg without issue. Sexual side effects are the most reported effects, though the reported incidence in trials is not dramatically different from placebo. Dutasteride has a much longer half-life than finasteride (weeks versus hours), so any side effect takes longer to resolve after stopping.

Uncommon (roughly 1 in 100)

• Reduced libido
• Erectile dysfunction
• Changes to ejaculation volume

Rare (less than 1 in 1,000)

• Breast tenderness or enlargement
• Mood changes (incidence unclear, still reported)
• Persistent sexual effects after stopping treatment

Very rare (less than 1 in 10,000)

• Reduced semen quality or male infertility (worth discussing with your prescriber if you are actively trying to conceive)
• Testicular pain or discomfort
• Changes in liver function

When to seek urgent medical advice

• Signs of allergic reaction: facial or lip swelling, difficulty breathing, rash
• Significant mood change or thoughts of self-harm
• Unusual breast lumps or nipple discharge

Who should not take dutasteride

• Women, in any circumstance
• Anyone under 18
• Anyone with a known allergy to dutasteride or other 5-alpha reductase inhibitors
• Anyone with severe liver disease should be reviewed carefully by a prescriber

Pregnancy and fertility

Dutasteride can affect a male foetus. The capsules are soft-gelatin coated, so simply taking dutasteride does not expose a pregnant partner. The risk arises only if a pregnant woman handles a leaking or damaged capsule. Handle the capsules with dry hands and do not split them.

Dutasteride can also be absorbed through skin, so blood donation from men on dutasteride is not accepted for at least 6 months after stopping. If you and your partner are actively trying to conceive, discuss with your prescriber, as dutasteride can reduce semen parameters in some men.

Warnings and precautions

• PSA lowering: Dutasteride can artificially lower Prostate Specific Antigen (PSA) blood test results by around 50%. If you have a PSA test in future for prostate cancer screening, tell your GP you are taking dutasteride so the reading can be doubled for accurate interpretation.
• Post-finasteride syndrome: If you have previously stopped finasteride due to persistent sexual, mood or physical effects, disclose this on the questionnaire so your prescriber can review carefully before starting dutasteride.
• Concurrent DHT-blocking treatment: Do not start oral dutasteride alongside oral finasteride, topical finasteride or topical dutasteride without prescriber guidance. Combining increases systemic exposure and side-effect risk.
• Longer half-life than finasteride: Effects and any side effects take longer to resolve on stopping, because dutasteride has a much longer half-life (weeks versus hours).
• Blood donation: Not eligible to donate blood while taking dutasteride or for 6 months after stopping.

Dutasteride vs finasteride

Finasteride 1mg is the licensed first-line UK treatment for male pattern hair loss. If it produces good results with no side effects, most men stay on it long term. Dutasteride is often chosen when:

• Finasteride has plateaued or produced only modest results
