How sildenafil works

Sildenafil is a phosphodiesterase type 5 (PDE-5) inhibitor. It relaxes the smooth muscle in the walls of blood vessels supplying the penis, allowing increased blood flow when you are sexually aroused. It does not cause an erection on its own; sexual stimulation is still required.

100mg is the maximum single dose of sildenafil. Many men are started at 50mg to assess response, and only step up to 100mg if 50mg is not effective enough. If you have not taken sildenafil before, say so in the health questionnaire so the prescriber can start you at the right strength.

How to take

• Take one tablet 30 to 60 minutes before you plan to have sex

• Effects typically last around 4 hours

• Do not take more than one dose in 24 hours

• Sexual arousal is still needed; sildenafil does not work without stimulation

• A heavy meal or excess alcohol before taking can delay or reduce the effect. Take on a relatively empty stomach for the best onset

• Avoid grapefruit and grapefruit juice, which can increase sildenafil blood levels

Side effects

Sildenafil is well tolerated by most men when taken as directed. Most side effects are mild and short-lived.

Very common (more than 1 in 10)

• Headache, often mild, and tends to resolve within 2 to 4 hours

Common (roughly 1 in 10)

• Facial flushing

• Indigestion or heartburn

• Nasal congestion

• Mild dizziness

• Blue-tinged vision or light sensitivity, temporary, resolving in 4 to 6 hours

Uncommon (roughly 1 in 100)

• Chest tightness

• Blurred vision

• Palpitations

• Skin rash

Rare (less than 1 in 1,000)

• Priapism, a painful erection lasting more than 4 hours. Medical emergency, see below

• Sudden hearing loss, usually in one ear. Medical emergency, see below

• Serious cardiovascular events including heart attack, stroke, or a dangerous drop in blood pressure

When to seek urgent medical advice

• Priapism: an erection lasting more than 4 hours needs A&E. Untreated priapism can cause permanent damage

• Sudden hearing loss or ringing in the ears: needs urgent review

• Chest pain, severe dizziness or fainting during or after sexual activity: call 999

• Sudden vision changes beyond a mild blue tint, such as dark spots or vision loss: needs urgent review

• Signs of allergic reaction: facial or lip swelling, difficulty breathing, widespread rash

Who should not take sildenafil

Sildenafil is not safe for everyone. Do not take it if you:

• Take any nitrate medicine such as GTN spray, isosorbide, amyl nitrite or "poppers". Combined use can cause a life-threatening drop in blood pressure

• Take riociguat or other guanylate cyclase stimulators

• Have had a heart attack, stroke, or serious heart-rhythm event in the last 6 months

• Have unstable angina, severe heart failure, or uncontrolled high or low blood pressure

• Have severe liver disease

• Have a hereditary eye condition such as retinitis pigmentosa

• Have previously had priapism, or have sickle-cell disease

• Are under 18 or female. Sildenafil is not licensed for these groups for ED

Drug interactions to disclose

Tell your prescriber if you take any of the following, because doses may need adjustment:

• Alpha-blockers, for blood pressure or the prostate, which need time-spacing

• Ritonavir, ketoconazole or itraconazole, which raise sildenafil blood levels significantly

• Erythromycin or clarithromycin

• Cimetidine, a heartburn medicine

• Other ED medicines such as tadalafil, vardenafil or avanafil

Generic sildenafil and branded Viagra

They are clinically the same medicine. Both contain sildenafil at the same strength, both are held to the same MHRA bioequivalence standard, and both produce the same clinical effect and the same side-effect profile. Generic sildenafil is standard practice in UK ED prescribing.

The honest differences are narrow:

• Manufacturer: this product is made by Milpharm. Branded Viagra is made by Pfizer, the original patent holder

• Recognition: some men specifically want the name they have heard of
