How sildenafil works

Sildenafil is a phosphodiesterase type 5 (PDE-5) inhibitor. It relaxes the smooth muscle in the walls of blood vessels supplying the penis, allowing increased blood flow when you are sexually aroused. It does not cause an erection on its own; sexual stimulation is still required.

  • Headache, often mild, and tends to resolve within 2 to 4 hours

Common (roughly 1 in 10)

  • Facial flushing

  • Indigestion or heartburn

  • Nasal congestion

  • Mild dizziness

  • Blue-tinged vision or light sensitivity, temporary, resolving in 4 to 6 hours

Uncommon (roughly 1 in 100)

  • Chest tightness

  • Blurred vision

  • Palpitations

  • Skin rash

Rare (less than 1 in 1,000)

  • Priapism, a painful erection lasting more than 4 hours. Medical emergency, see below

  • Sudden hearing loss, usually in one ear. Medical emergency, see below

  • Serious cardiovascular events including heart attack, stroke, or a dangerous drop in blood pressure

When to seek urgent medical advice

  • Priapism: an erection lasting more than 4 hours needs A&E. Untreated priapism can cause permanent damage

  • Sudden hearing loss or ringing in the ears: needs urgent review

  • Chest pain, severe dizziness or fainting during or after sexual activity: call 999

  • Sudden vision changes beyond a mild blue tint, such as dark spots or vision loss: needs urgent review

  • Signs of allergic reaction: facial or lip swelling, difficulty breathing, widespread rash

Who should not take sildenafil

Sildenafil is not safe for everyone. Do not take it if you:

  • Take any nitrate medicine such as GTN spray, isosorbide, amyl nitrite or "poppers". Combined use can cause a life-threatening drop in blood pressure

  • Take riociguat or other guanylate cyclase stimulators

  • Have had a heart attack, stroke, or serious heart-rhythm event in the last 6 months

  • Have unstable angina, severe heart failure, or uncontrolled high or low blood pressure

  • Have severe liver disease

  • Have a hereditary eye condition such as retinitis pigmentosa

  • Have previously had priapism, or have sickle-cell disease

  • Are under 18 or female. Sildenafil is not licensed for these groups for ED

Drug interactions to disclose

Tell your prescriber if you take any of the following, because doses may need adjustment:

  • Alpha-blockers, for blood pressure or the prostate, which need time-spacing

  • Ritonavir, ketoconazole or itraconazole, which raise sildenafil blood levels significantly

  • Erythromycin or clarithromycin

  • Cimetidine, a heartburn medicine

  • Other ED medicines such as tadalafil, vardenafil or avanafil
