Finasteride 5mg Tablets (off-label for hair loss)

Finasteride 5mg is licensed for benign prostatic hyperplasia (BPH), not for hair loss. Some UK prescribers use it off-label for male pattern hair loss at a reduced dose, because the evidence shows no added hair-loss benefit above 1mg and the 5mg tablet can be the lowest-cost route. How finasteride works for hair loss

Finasteride is a Type II 5-alpha reductase inhibitor. It blocks the enzyme that converts testosterone into dihydrotestosterone (DHT), the hormone responsible for miniaturising hair follicles in men with male pattern hair loss.

The licensed hair-loss dose is 1mg daily, which reduces serum DHT by around 70%. Because the dose-response curve plateaus around 1mg, there is no hair-loss benefit to dosing above it, which is why the cheaper 5mg tablet can be prescribed off-label at a reduced dose. This is well-established off-label practice, supported by pharmacokinetic evidence that even lower doses (0.2mg) produce meaningful DHT suppression.

Dosing

The dose and how to take it are set by your prescriber for you individually and confirmed at prescriber review. This is not a self-managed product. Finasteride 5mg is licensed for BPH, not hair loss, so any hair-loss use is off-label and the prescriber decides the strength and schedule.

What to expect

• 2 to 3 months: reduced shedding is often the first sign

• 4 to 6 months: visible thickening and some regrowth on the crown and mid-scalp

• 12 months: full effect is judged. Results plateau and are maintained by continued daily use

• On stopping: any hair gained is lost over 6 to 12 months as DHT returns to normal

Side effects

Most men take finasteride without issue. In the trials supporting the licence, at 12 months against placebo: decreased libido 1.8% versus 1.3%, erectile dysfunction 1.3% versus 0.7%, and ejaculation disorder 0.8% versus 0.4%. By year five each had fallen below 0.3%.

Uncommon (roughly 1 in 100)

• Reduced libido, usually mild

• Erectile dysfunction

• Changes to ejaculation volume

Rare (less than 1 in 1,000)

• Breast tenderness or enlargement

• Mood changes (incidence unclear, still reported)

• Persistent sexual effects after stopping treatment

Very rare (less than 1 in 10,000)

• Reduced semen quality or male infertility (worth discussing with your prescriber if you are actively trying to conceive)

• Testicular pain or discomfort

• Changes in liver function

When to seek urgent medical advice

• Signs of allergic reaction: facial or lip swelling, difficulty breathing, rash

• Significant mood change or thoughts of self-harm

• Unusual breast lumps or nipple discharge

Who should not take finasteride

• Anyone who is pregnant, breastfeeding, or trying to conceive

• Anyone under 18

• Anyone with a known allergy to finasteride

• Anyone with severe liver disease should be reviewed carefully

Pregnancy and fertility

Finasteride can affect a male foetus. The tablets are film-coated, so simply taking finasteride does not expose a pregnant partner. The risk arises only if a pregnant woman handles a broken or crushed tablet. Store the pack securely. If your prescriber has directed you to split tablets, do so away from anyone who is pregnant and wash your hands afterwards.

Finasteride is licensed for men. It is sometimes prescribed to women off-label, at the prescriber's discretion, and only where there is no possibility of pregnancy. It cannot be prescribed to anyone who is pregnant, breastfeeding, or trying to conceive. Your prescriber reviews this from your questionnaire answers.

Warnings and precautions

• Post-finasteride syndrome: if you have previously stopped finasteride due to persistent sexual, mood or physical effects, disclose this on the questionnaire so your prescriber can review carefully

• Concurrent DHT-blocking treatment: do not combine with dutasteride or topical finasteride/dutasteride without prescriber guidance. Combining increases systemic exposure and side-effect risk

• PSA screening: finasteride at 5mg (the BPH dose) is known to lower PSA blood test results by around 50%. At the reduced doses used for hair loss the PSA effect is smaller but still present. Tell your GP you take finasteride if you have a PSA test

• Fertility planning: if you and your partner are actively trying to conceive, discuss with your prescriber before starting

5mg off-label vs licensed 1mg tablets

The licensed hair-loss product is finasteride 1mg daily. The 5mg tablet is licensed for enlarged prostate and is used off-label for hair loss at a reduced dose, which can lower the cost per month. Some men prefer the licensed 1mg for simplicity; others are prescribed the 5mg pack for value. Both are legitimate options prescribed regularly in UK practice, and which one suits you is decided at prescriber review.

Clinical evidence: what off-label 5mg prescribing is built on

The evidence base for finasteride in male pattern hair loss is 25+ years deep. The dose-response question specifically is where off-label 5mg prescribing comes in.

• Roberts et al 1999 compared finasteride 0.2mg, 1mg, 5mg and placebo daily in men with androgenetic alopecia over 12 months. The 1mg and 5mg doses produced clinically similar hair count improvements (both significantly better than 0.2mg and placebo). The practical point: 5mg does not out-perform 1mg for hair loss, so there is no benefit to dosing above 1mg.
• Original Merck 5 year trials (1998-2003) supporting the MHRA 1mg licence found 80 to 90% of men maintain or improve hair count at 2 years on 1mg daily. This is the outcome bar every finasteride option is measured against.
• DHT suppression at 5mg vs 1mg: serum DHT is reduced by ~70% at 1mg daily and ~70% at 5mg daily. The dose-response plateau in DHT reduction is why the two doses produce similar outcomes for hair growth.

Practical outcome: because DHT suppression plateaus around 1mg, the cheaper 5mg tablet can be prescribed off-label at a reduced dose for hair loss rather than the licensed 1mg strength. The dose is set by the prescriber for the individual.

Regulatory status in the UK: 5mg is BPH-licensed, off-label for hair loss

• Finasteride 5mg is MHRA licensed in the UK under the brand name Proscar and its generic equivalents, for benign prostatic hyperplasia (BPH) in older men.
• For hair loss, the MHRA licensed strength is 1mg daily (Propecia and generics). 5mg used for hair loss is an off-label prescribing decision.
• Off-label prescribing is legal and common practice in the UK when the prescriber judges it appropriate for the individual patient. The evidence base above is why UK prescribers accept off-label 5mg prescribing for hair loss when cost is a decision factor.

Storage and shelf life

• Store at room temperature (below 25°C), away from direct sunlight and moisture.
• Keep tablets in their original blister pack or bottle until you take them. Do not decant into a weekly pill organiser more than 7 days ahead.
• Sealed pack expiry is on the label (typically 24 to 36 months from manufacture). Do not use after the printed expiry date.
• If your prescriber has directed you to split tablets, store the cut pieces in the original bottle with the cap closed and use them within 2 weeks, as the cut edge can oxidise once exposed to air.
• Keep out of reach of children and pets, and keep away from anyone pregnant or planning pregnancy (see pregnancy and fertility section above).

Excipients: what is in each 5mg tablet

Active ingredient: finasteride 5mg. Typical excipients in UK generic finasteride 5mg tablets:

• Lactose monohydrate as the primary filler. If you are lactose intolerant, ask your prescriber about lactose-free finasteride tablet brands.
• Microcrystalline cellulose, pregelatinised starch, sodium starch glycolate, magnesium stearate as tablet binders and lubricants.
• Docusate sodium as a wetting agent.
• Talc, hypromellose, titanium dioxide in the film coating for colour and dissolution timing.
