Finasteride 5mg Tablets (Split-Dose for Hair Loss)

How finasteride works for hair loss

Finasteride is a Type II 5-alpha reductase inhibitor. It blocks the enzyme that converts testosterone into dihydrotestosterone (DHT), the hormone responsible for miniaturising hair follicles in men with male pattern hair loss.

The licensed hair-loss dose is 1mg daily, which reduces serum DHT by around 70%. Taking a split fraction of a 5mg tablet delivers a similar hair-loss dose at a lower cost per month. This is a well-established off-label practice supported by pharmacokinetic evidence that even lower doses (0.2mg) produce meaningful DHT suppression.

Dosing

The 5mg tablet is scored and typically split for hair-loss dosing. Standard UK approach:

• Half tablet daily (2.5mg): the most common split-dose approach. Delivers robust DHT suppression, close to what 1mg daily produces.

• Half tablet 3 times per week (2.5mg 3x/week): lower total exposure while still meaningful. Often used by men who want the minimum effective dose.

A quality pill splitter is essential for accurate halves. Take at any time of day, with or without food. Consistency matters more than timing.

What to expect

• 2 to 3 months: reduced shedding is often the first sign

• 4 to 6 months: visible thickening and some regrowth on the crown and mid-scalp

• 12 months: full effect is judged. Results plateau and are maintained by continued daily use

• On stopping: any hair gained is lost over 6 to 12 months as DHT returns to normal

Side effects

Most men take split-dose finasteride without issue. Original phase III trials at 1mg daily reported sexual side effects in around 3 to 4% of men, with a similar rate in the placebo arm.

Uncommon (roughly 1 in 100)

• Reduced libido, usually mild

• Erectile dysfunction

• Changes to ejaculation volume

Rare (less than 1 in 1,000)

• Breast tenderness or enlargement

• Mood changes (incidence unclear, still reported)

• Persistent sexual effects after stopping treatment

Very rare (less than 1 in 10,000)

• Reduced semen quality or male infertility (worth discussing with your prescriber if you are actively trying to conceive)

• Testicular pain or discomfort

• Changes in liver function

When to seek urgent medical advice

• Signs of allergic reaction: facial or lip swelling, difficulty breathing, rash

• Significant mood change or thoughts of self-harm

• Unusual breast lumps or nipple discharge

Who should not take finasteride

• Women, in any circumstance

• Anyone under 18

• Anyone with a known allergy to finasteride

• Anyone with severe liver disease should be reviewed carefully

Pregnancy and fertility

Finasteride can affect a male foetus. The tablets are film-coated, so simply taking finasteride does not expose a pregnant partner. The risk arises only if a pregnant woman handles a broken or crushed tablet. Store the pack securely, split tablets away from anyone pregnant, and wash your hands after handling.

Warnings and precautions

• Post-finasteride syndrome: if you have previously stopped finasteride due to persistent sexual, mood or physical effects, disclose this on the questionnaire so your prescriber can review carefully

• Concurrent DHT-blocking treatment: do not combine with dutasteride or topical finasteride/dutasteride without prescriber guidance. Combining increases systemic exposure and side-effect risk

• PSA screening: finasteride at 5mg (the BPH dose) is known to lower PSA blood test results by around 50%. At split hair-loss doses the PSA effect is smaller but still present. Tell your GP you take finasteride if you have a PSA test

• Fertility planning: if you and your partner are actively trying to conceive, discuss with your prescriber before starting

Clinical evidence: what the split-dose approach is built on

The evidence base for finasteride in male pattern hair loss is 25+ years deep. The dose-response question specifically is where split-dose 5mg comes in.

• Roberts et al 1999 compared finasteride 0.2mg, 1mg, 5mg and placebo daily in men with androgenetic alopecia over 12 months. The 1mg and 5mg doses produced clinically similar hair count improvements (both significantly better than 0.2mg and placebo). This is the foundation for the split-dose approach: 5mg does not out-perform 1mg for hair loss, so higher doses are not needed to grow hair.
• Original Merck 5 year trials (1998-2003) supporting the MHRA 1mg licence found 80 to 90% of men maintain or improve hair count at 2 years on 1mg daily. This is the outcome bar every finasteride option is measured against.
• DHT suppression at 5mg vs 1mg: serum DHT is reduced by ~70% at 1mg daily and ~70% at 5mg daily. The dose-response plateau in DHT reduction is why the two doses produce similar outcomes for hair growth.

Practical outcome: 5mg split into thirds (roughly 1.67mg per day) produces the same scalp DHT suppression as a 1mg tablet at roughly one-third the monthly cost. This is why UK prescribers sometimes prescribe 5mg off-label for cost-conscious patients rather than the licensed 1mg strength.

Regulatory status in the UK: 5mg is BPH-licensed, off-label for hair loss

• Finasteride 5mg is MHRA licensed in the UK under the brand name Proscar and its generic equivalents, for benign prostatic hyperplasia (BPH) in older men.
• For hair loss, the MHRA licensed strength is 1mg daily (Propecia and generics). 5mg used for hair loss is an off-label prescribing decision.
• Off-label prescribing is legal and common practice in the UK when the prescriber judges it appropriate for the individual patient. The evidence base above is why UK prescribers accept the split-dose 5mg approach for hair loss when cost is a decision factor.

Storage and shelf life

• Store at room temperature (below 25°C), away from direct sunlight and moisture.
