How dutasteride works

Dutasteride is a dual 5-alpha reductase inhibitor. It blocks both Type I and Type II 5-alpha reductase enzymes that convert testosterone into dihydrotestosterone (DHT), reducing serum DHT by around 90 to 98% at the 0.5mg daily dose. Compared with finasteride (which blocks only Type II and reduces DHT by around 70%), dutasteride produces more complete DHT suppression.

What to expect

  • 2 to 3 months: reduced shedding is often the first sign
  • 4 to 6 months: visible thickening and some regrowth on the crown and mid-scalp
  • 12 months: full effect. Results maintained by continued daily use
  • On stopping: any hair gained is lost over 6 to 12 months. Dutasteride has a long half-life (roughly 5 weeks) so DHT levels take longer to recover than after finasteride

Side effects

Most men take dutasteride 0.5mg without issue. Dutasteride has a much longer half-life than finasteride (weeks versus hours), so any side effect takes longer to resolve after stopping.

Uncommon (roughly 1 in 100)

  • Reduced libido
  • Erectile dysfunction
  • Changes to ejaculation volume

Rare (less than 1 in 1,000)

  • Breast tenderness or enlargement
  • Mood changes (incidence unclear, still reported)
  • Persistent sexual effects after stopping treatment

Very rare (less than 1 in 10,000)

  • Reduced semen quality or male infertility (worth discussing with your prescriber if you are actively trying to conceive)
  • Testicular pain or discomfort
  • Changes in liver function

When to seek urgent medical advice

  • Signs of allergic reaction: facial or lip swelling, difficulty breathing, rash
  • Significant mood change or thoughts of self-harm
  • Unusual breast lumps or nipple discharge

Warnings and precautions

  • PSA lowering: Dutasteride can artificially lower Prostate Specific Antigen (PSA) blood test results by around 50%. If you have a PSA test in future for prostate cancer screening, tell your GP you are taking dutasteride so the reading can be doubled for accurate interpretation.
  • Post-finasteride syndrome: If you have previously stopped finasteride due to persistent sexual, mood or physical effects, disclose this on the questionnaire so your prescriber can review carefully.
  • Concurrent DHT-blocking treatment: Do not combine with oral finasteride, topical finasteride or topical dutasteride without prescriber guidance.
  • Longer half-life: Effects and any side effects take longer to resolve on stopping (weeks versus hours for finasteride).
  • Blood donation: Not eligible to donate blood while taking dutasteride or for 6 months after stopping.

  • Women, in any circumstance
  • Anyone under 18
  • Anyone with a known allergy to dutasteride or other 5-alpha reductase inhibitors
  • Anyone with severe liver disease should be reviewed carefully by a prescriber

How does dutasteride compare to finasteride?+

Dutasteride is a stronger DHT blocker: 90 to 98% suppression vs finasteride 70%. It blocks both Type I and Type II 5-alpha reductase, whereas finasteride blocks only Type II. In clinical trials this produces modestly better hair count and density, particularly at the crown.
