Dutasteride + Minoxidil Combination Capsules (100 Capsules)

Prescription oral capsule combining dutasteride 0.5mg with low-dose minoxidil (2.5mg or 5mg) in a single daily capsule, so the two-drug hair loss regimen becomes one capsule a day. Compounded to order in the UK in vegetarian HPMC shells (gelatine free). 100 capsules per bottle, roughly 3 months plus 10 days at once-daily dosing.

How the two parts work

  • Dutasteride is a dual 5-alpha reductase inhibitor. 5-alpha reductase exists in two forms. Finasteride inhibits type II only and reduces serum DHT by around 70%. Dutasteride inhibits both type I and type II, reducing serum DHT by around 90 to 98% at the 0.5mg daily dose. DHT is the hormone that miniaturises hair follicles.

  • Minoxidil acts on the microcirculation around the follicle and prolongs the growth (anagen) phase of the hair cycle.

The two act on different parts of the hair cycle: one on the hormonal signal, one on the follicle itself. Putting both into a single capsule removes the need to remember two separate medicines.

Vegetarian capsules

Standard dutasteride capsules use bovine gelatine. These are filled into vegetarian HPMC shells, made from a plant cellulose rather than gelatine. The actives and the doses are unchanged; only the shell material differs, so absorption and clinical effect are the same.

How to take

One capsule daily, at any time of day, with or without food. Consistency matters more than timing. If you miss a dose, take it as soon as you remember on the same day; do not double up the next day.

What to expect

  • 2 to 3 months: reduced shedding is often the first sign

  • 4 to 6 months: any visible change on the crown and mid-scalp is assessed over this period with your prescriber

  • 12 months: full effect. Any result is maintained by continued daily use

  • Initial shedding in the first 4 to 8 weeks is normal and comes from the minoxidil component

  • On stopping: hair gained is lost over 6 to 12 months. Dutasteride has a long half-life (roughly 5 weeks), so DHT levels take longer to recover than after finasteride

Side effects

Because this combines two medicines, side effects of both are possible. Most men take the combination without issue.

Exact frequencies are not established for a compounded capsule, and the rates quoted for the licensed dutasteride capsules and minoxidil tablets do not carry across, so the list below is ordered by likelihood rather than precise figures.

Dutasteride related: less likely

  • Reduced libido, usually mild

  • Erectile dysfunction

  • Changes to ejaculation volume

Dutasteride related: unlikely

  • Breast tenderness or enlargement

  • Mood changes (incidence unclear, still reported)

  • Persistent sexual effects after stopping

  • Reduced semen quality (worth discussing with your prescriber if you are actively trying to conceive)

Dutasteride has a much longer half-life than finasteride (weeks rather than hours), so any dutasteride related effect takes longer to resolve after stopping.

Minoxidil related: more likely

  • Increased body or facial hair (hypertrichosis). Usually mild in men, most often on the arms

  • Initial hair shedding in the first 4 to 8 weeks. Temporary and normal

Minoxidil related: less likely

  • Ankle swelling (fluid retention)

  • Lightheadedness on standing

  • Modest increase in resting heart rate

When to seek urgent medical advice

  • Signs of allergic reaction: facial or lip swelling, difficulty breathing, rash

  • Persistent chest pain, palpitations or breathlessness

  • Sudden significant ankle or leg swelling

  • Significant mood change or thoughts of self-harm

  • Unusual breast lumps or nipple discharge

Warnings and precautions

  • PSA lowering: dutasteride can artificially lower Prostate Specific Antigen (PSA) blood test results by around 50%. If you have a PSA test in future for prostate cancer screening, tell your GP you are taking dutasteride so the reading can be doubled for accurate interpretation.

  • Longer half-life: effects and any side effects take longer to resolve on stopping (weeks rather than hours for finasteride).

  • Concurrent DHT blocking treatment: do not combine with oral finasteride, topical finasteride or topical dutasteride without prescriber guidance.

  • Other minoxidil: tell your prescriber if you already use topical minoxidil, so the total dose can be reviewed.

  • Post-finasteride syndrome: if you have previously stopped finasteride because of persistent sexual, mood or physical effects, disclose this on the questionnaire so your prescriber can review carefully.

  • Blood donation: not eligible to donate blood while taking dutasteride or for 6 months after stopping.

Who should not take this combination

  • Anyone who is pregnant, breastfeeding, or trying to conceive

  • Anyone under 18

  • Known allergy to dutasteride, to other 5-alpha reductase inhibitors, or to minoxidil

  • Uncontrolled high blood pressure or recent heart attack (minoxidil concern)

  • History of significant cardiac arrhythmia or pre-existing heart disease (needs prescriber review)

  • Concurrent blood pressure medication (needs prescriber review)

  • Severe liver disease (needs specialist review)

Pregnancy and fertility

Dutasteride can affect a male foetus. The capsules should not be handled by anyone pregnant, and broken capsules should not be handled at all. If you are actively trying to conceive with your partner, discuss this with your prescriber before starting; dutasteride can reduce semen parameters in some men.

Dutasteride is licensed for men. It is sometimes prescribed to women off-label, at the prescriber’s discretion, and only where there is no possibility of pregnancy.
