Dutasteride for Hair Loss: How It Works, Who It Is For, and What to Expect

Dutasteride is the stronger of the two 5-alpha reductase inhibitors used for male pattern hair loss. Reduces serum DHT by 90 to 98 percent versus roughly 70 percent for finasteride, because it blocks both Type I and Type II 5-alpha reductase enzymes rather than just Type II. Dutasteride brands available in the UK

Every UK dutasteride 0.5mg capsule contains the same active at the same dose. The manufacturer, capsule appearance and price differ. Any UK prescriber will confirm these are clinically interchangeable.

• Avodart (GSK, later Aspen Pharma): the original branded dutasteride. First MHRA licensed in the UK in 2003 for benign prostatic hyperplasia (BPH). Distinctive yellow soft gelatin capsule with "GX CE2" printed on it. Typically £25 to £40 per 30 capsules at UK retail. See our Avodart 0.5mg.

• Combodart (GSK): a combination of dutasteride 0.5mg plus tamsulosin 0.4mg licensed for BPH. Not prescribed for hair loss (the tamsulosin adds no benefit for hair and unnecessary side effects).

How dutasteride works for hair loss

Dutasteride is a dual 5-alpha reductase inhibitor. In plain English: the enzyme 5-alpha reductase (in two forms, Type I and Type II) converts testosterone into dihydrotestosterone (DHT), the hormone that shrinks hair follicles in male pattern hair loss. Dutasteride blocks BOTH forms of the enzyme, whereas finasteride blocks only Type II. Less enzyme activity means less DHT, means less follicle miniaturisation.

Key mechanism numbers:

• Dutasteride 0.5mg daily reduces serum DHT by 90 to 98 percent. Finasteride 1mg reduces it by roughly 70 percent.
• Scalp DHT reduction is similarly stronger on dutasteride.
• Half-life is roughly 5 weeks (versus 6 to 8 hours for finasteride). Effects and side effects both take longer to resolve on stopping.
• Effect on serum testosterone is minimal, similar to finasteride.

The longer half-life is a clinically important point. If you start dutasteride and develop side effects, it takes weeks to months for the drug to clear versus a few days for finasteride. Prescribers factor this in when comparing the two.

Licensed vs off-label uses in the UK

• Dutasteride 0.5mg is MHRA licensed in the UK for benign prostatic hyperplasia (BPH) in older men, under Avodart and generic equivalents.
• Dutasteride for male pattern hair loss is OFF-LABEL in the UK. Unlike finasteride 1mg (which is on-label for hair loss under Propecia and generics), dutasteride has never been granted a specific hair loss licence in the UK.
• Off-label prescribing is legal and common practice when the prescriber judges it appropriate for the individual patient. The evidence base (below) is why UK prescribers accept dutasteride for hair loss, particularly as an escalation from finasteride 1mg that has plateaued.
• In South Korea and Japan, dutasteride IS licensed for male pattern hair loss, so a specific hair loss indication does exist in some markets. UK MHRA has just never granted it.

Clinical evidence: the trials behind dutasteride for hair loss

• Olsen et al 2006 (Phase III, 416 men, 24 weeks): compared dutasteride 0.1mg, 0.5mg, 2.5mg and finasteride 5mg. Dutasteride 0.5mg produced hair count improvements significantly better than finasteride 5mg (which itself matches 1mg for hair loss). Total hair count gain at 24 weeks was around 92 hairs per 2.54cm² area at the vertex on dutasteride 0.5mg vs 41 hairs on finasteride 5mg vs 3 hairs on placebo.
• Gubelin Harcha et al 2014 (Phase III South Korea/Taiwan, 917 men, 24 weeks): larger head-to-head confirmation of dutasteride 0.5mg superiority over finasteride 1mg for both hair count and Global Photographic Assessment. This is the trial that supported dutasteride licensing for hair loss in South Korea and Japan.
• Choi et al 2016 long term follow-up: tracked dutasteride 0.5mg daily out to 3 years in Korean men. Sustained gains through year 3, no evidence of loss of effect. Non-responder rate at 3 years was lower than for finasteride 1mg.
• Zhou et al 2018 meta-analysis: pooled 6 RCTs comparing dutasteride vs finasteride for hair loss. Confirmed dutasteride produces roughly 25 percent greater hair count improvement at 24 weeks, with similar side effect profile.

Practical takeaway: dutasteride is more effective than finasteride at hair count, but not licensed for the indication in the UK. Prescribers typically reach for it when finasteride 1mg has plateaued or the pattern of loss is aggressive. Both work; dutasteride is stronger.

How long dutasteride takes to work

• Weeks 1 to 4: no visible change. Serum DHT falling by 90 to 98 percent but hair does not respond in real time.
• Months 3 to 6: reduced shedding is the first visible sign.
• Months 6 to 12: visible hair count and thickness improvements. Full effect judged at 12 months.
• Months 12 to 36: continued gradual improvement in responders. Peak effect at 2 to 3 years.
• On stopping: because of the 5 week half-life, DHT takes 3 to 6 months to fully return to baseline, and any hair gains are lost gradually over 12 to 18 months. Longer washout than finasteride.

Consistency matters. Missing one day is fine; the 5 week half-life buffers you. Missing multiple weeks progressively erodes the DHT suppression that hair growth depends on.

Side effects: the honest picture

• Sexual side effects (reduced libido, erectile dysfunction, ejaculation disorders): reported in 3 to 6 percent of men on dutasteride 0.5mg daily in the Olsen 2006 trial, slightly higher than the 2 to 4 percent reported for finasteride 1mg. Usually mild and reversible, but the 5 week half-life means resolution on stopping takes longer than for finasteride.
• Breast tenderness or enlargement (gynecomastia): uncommon, roughly 1 percent incidence. Usually resolves on stopping.
• Mood changes (low mood, depression, anxiety): reported in post-marketing surveillance. Report to your prescriber if new-onset.
• PSA test effect: dutasteride reduces PSA (prostate specific antigen) by roughly 50 percent. Any PSA reading while on dutasteride should be DOUBLED to compare against normal reference ranges. Critical for men undergoing prostate cancer screening.
• Blood donation: not eligible to donate blood while on dutasteride, or for 6 months after stopping. This is a UK NHS Blood and Transplant rule.
• Post-finasteride/dutasteride syndrome (PFS): a small subset of men report persistent sexual, mood or physical side effects that continue after stopping. The published incidence is low, but the mechanism is not fully understood. UK prescribers acknowledge PFS as a real (though uncommon) concern. If you develop side effects, tell your prescriber immediately rather than waiting; early discontinuation is associated with better resolution outcomes.

Dutasteride vs the alternatives

• Dutasteride 0.5mg oral (this guide): the strongest DHT blocker available. Off-label in the UK. Typically reserved for men who plateaued on finasteride or need aggressive DHT block.

What NOT to combine: never take dutasteride with finasteride (two DHT blockers, no meaningful additional benefit, higher side effect risk, no supporting evidence). Never take oral dutasteride with topical dutasteride (same drug, two routes stacks the dose unnecessarily). Minoxidil plus dutasteride is fine and encouraged; different drugs, complementary mechanisms.

Dosage

The standard hair loss dose is one 0.5mg capsule daily, at any consistent time, with or without food. This matches the BPH licensed dose. Higher off-label doses (1mg, 2.5mg) have been studied but do not out-perform 0.5mg for hair count in published trials. Take one capsule daily; do not open, break or chew the capsule.

Some prescribers use lower doses (0.25mg every other day, or 0.5mg every other day) for men who tolerated finasteride but developed mild side effects on daily dutasteride. This is off-label dosing on top of off-label indication; discuss with your prescriber.

Storage and shelf life

• Store at room temperature (below 30°C), away from direct sunlight and moisture.
• Do NOT store in a bathroom cabinet. Heat and humidity degrade the soft gelatin capsule shell over time.
• Keep capsules in the original blister pack until you take them.
• Sealed pack expiry is printed on the pack (typically 24 to 36 months from manufacture). Do not use after the printed expiry date.
• Do NOT open, break, chew or crush the capsules. Dutasteride can be absorbed through the skin, and pregnant women or women who may be pregnant should NEVER handle broken capsules. Broken or leaking capsules must be handled with care and washed off the skin immediately if contact occurs.
• Keep out of reach of children, pets, and anyone pregnant or planning pregnancy.

Excipients: what is in each Avodart or generic dutasteride 0.5mg soft capsule

Active: dutasteride 0.5mg. Avodart is a soft gelatin capsule, so excipients differ from typical tablet fillers. Typical composition:

• Capsule fill: mono- and di-glycerides of caprylic/capric acid (a fatty vehicle that dissolves the dutasteride), plus butylated hydroxytoluene (BHT) as an antioxidant to keep the drug stable.
• Capsule shell: gelatin, glycerol, titanium dioxide, iron oxide yellow (E172) giving the capsule its characteristic yellow colour.
• Printing ink: shellac, propylene glycol, iron oxide red (E172) for the "GX CE2" branding.

The capsule is NOT vegetarian or vegan because the shell contains gelatin. If dietary requirements matter, ask your prescriber about generic dutasteride tablet formulations, which may have different excipients. Full excipient list on the manufacturer PIL enclosed with your medicine.

Regulatory status in the UK

• Avodart 0.5mg has been MHRA licensed in the UK since 2003 for benign prostatic hyperplasia. Generic dutasteride 0.5mg capsules are MHRA licensed to the same clinical file.
